Enrolment form

Please have the following documents ready to upload:

  1. Birth Certificate or passport,
  2. Immunisation certificate,
  3. Proof of address (utility bill), and
  4. Any parenting/custody orders.

Tip: if you don’t have a scanner, take a picture with your smartphone
